Does your van happen to have trailer lights - could be the adapter is screwing up. Also could mean your alternator is dying. Both have happened to me causing the same type of gremlins. I have been trying to track what is shorting out my left headlights and tail lights. If there is a direct correlation I am not sure but my dash lights are out as well as horn. If you are having problems with the window switch I would try to remove and the switch take it apart and clean all of the terminals and connections in the switch. I wouldn't say that 92 and newer have the 134a coolants but I can say that I have a 1991 Delica that had already been converted from the R-12 to 134a when it came over from Japan.
