Grinding noise + smoke

Hey folks,

So im having a issue with my l300 with the 4d56. ill give you a little back story before i dive right in. I have owned this van for a little over 2 years. I bought it with 108000km and i am currently just a touch over 150000km so i i have put on about 42000km since i have owned it. I have never had any problems with the van since i have owned it. the only work i have done is the basic stuff like oil change, glow plugs etc. I also changed the timing belt about a year ago when my other belt went.

The problem happened today. My van was parked at my acreage and with the recent rain it was a touch muddy. I couldnt get out in 2wd so I put it in 4wd. I engaged and i crawled out with no issues and then put it back into 2wd (I should note that i probly haven't used 4wd in more then 6 months) and continued on. once i got on the highway and tried getting it to speed I could tell something was wrong. I lost most all my power once i hit 80km/h. The power was cutting in and out in and out and then i could here some grinding noise. once i pulled over and lift the seat up there was a little bit of smoke coming up from right where the upper rad hose connects to the block. couldnt tell if it was the hose itself or was coming from below. I tried re engaging and disengaging the 4wd again to see if that helped but no luck. It seems to only make the grinding noise when im on the throttle. A "mechanic" ( at least thats what he told me) pulled over and told me he things its the timing tension-er or something but im not convinced. Im no diesel mechanic so I really am not to sure what im dealing with.
